ONE AXIS · Perioperative Value Transformation

From Surgical Volume to Surgical Value.

We redesign surgical care to create measurable clinical, operational and financial value.

Better Outcomes. Better Flow. Better Economics.

Executive tension

More surgical volume does not automatically create more value.

Hospitals know their clinical, operational and financial indicators. Yet each lives on a different axis — quality, flow, finance — and the decisions that create or destroy value happen between them: in handoffs, in variation, in resource use, in reimbursement, in what is done with capacity. Volume grows; value does not follow by default.

Blind spot

Value can disappear between the indicators you already measure.

Length of stay, ICU use, complications, cost per case, contribution margin — each is measured somewhere. Perioperative value leakage happens in the space between them: avoidable days, unwarranted variation, capacity that is released but never redeployed, savings that accrue to someone else. Nobody owns the gap, so nobody sees it.

DecisionPre-opAdmissionORPACU / ICUWardDischargeRecoveryValue leaks between the steps — in handoffs, variation, resource use and capacity decisions.

Methodology

From insight to measurable change.

A standardized method with pathway-specific clinical content: Measure → Diagnose → Redesign → Implement → Prove. Every engagement reports five public Value Deltas — Clinical, Implementation, Flow, Economic and Capacity — each with its baseline, its denominator and its owner.

Capacity

Released capacity creates optionality.

When a pathway improves, beds, ICU hours and operating-room time are released. Whether that becomes value depends on a management decision: GROW into additional activity, RESIZE structural cost, AVOID future investment, or BUFFER for resilience. ONE AXIS quantifies the options. Management decides how capacity should create value.

Economics

Creating value and capturing value are not the same thing.

Who benefits when a surgical journey improves depends on the reimbursement model, on demand and on capacity reality. Under one commercial model the hospital keeps the financial gain; under another, the payer does. ONE AXIS makes economic alignment explicit before anyone is asked to change — so that the institution that does the work is not the one that loses.

Evidence

Built from real-world perioperative transformation experience.

ONE AXIS is founded on prior founder-led pathway transformations across multiple surgical lines in a large private hospital — documented with baselines, denominators and cost data. That experience informs the methodology; future ONE AXIS projects build the corporate evidence base. We publish what we can prove and label what we cannot.

Leadership

Founder-led. System-driven.

ONE AXIS was founded by Prof. Dr. Luiz Fernando dos Reis Falcão, a physician, executive and academic leader in anesthesiology and perioperative medicine who has led surgical pathway transformations from the inside of hospitals. The Founder opens the conversation; the ONE AXIS system — method, evidence discipline and governance — does the work.
